The Cleveland Department of Public Health conducts inspection visits to ensure that healthcare facilities providing abortion services maintain a
safe, clean, and regulatory-compliant environment for medical procedures. Our goal is to safeguard the health and well-being of women receiving care in these facilities.
Annual Re-Licensure Process
As part of the re-licensure process, a
public health nurse will conduct an on-site visit to observe operations, review patient charts, and meet with select staff. Compliance with
City Codes may also be verified by
fire and building inspectors.
Upon successful completion of inspections and receipt of the
$260 licensing fee, your
2025 license will be issued.
Steps for Renewing an Abortion Services License
- Step 1: Application for Licensure
Submit the completed application along with the $260 license fee by November 1, 2025, to:

Return the completed questionnaire by November 1, 2025, to:

Once your application and renewal questionnaire are received, your facility will be scheduled for an on-site inspection.
